Once you have completed the short survey, click Submit and a second form will appear, in which you will enter the information you would like to have engraved on the free heirloom plate.
Free heirloom plates are only available for select floor clocks. Engraved plates are not available for some floor clock models, and all wall, mantel and table clocks.
